CET: Certificate of Capacity Forms

NOTICE FROM THE C.E. Asbestos Thurston TRUST SEPTEMBER 29, 2021: The C.E. Thurston Asbestos Trust (“CET”) adopts the following two forms related to the certificate of capacity requirement.  If required, a claim’s certificate of capacity requirement may be satisfied by the submission of the following two forms, available on the CET website:

    • Law Firm/Attorney’s Representations Affirming Official Representative’s Authority
    • Affidavit of Decedent’s Representative
